Jenesis Living LLC is an Iowa Medicaid-enrolled HCBS provider specializing in Home Based Habilitation. We value prompt communication, professional collaboration, person-centered planning, honest discussions about service fit, and transparency throughout the review process—always in partnership with the individual’s broader support team.

Current services

Home Based Habilitation (HBH) is the service we provide. It is delivered through two residential models—not as separate clinical services.

Home Based Habilitation (HBH)

Host Home Services

Support in a welcoming home environment with stable daily routines and relationship-based care.

Staffed Home Services

Small residential settings with structured daily support from direct support professionals.

Some situations need additional discussion before services can be offered. This is not an automatic exclusion list—each referral is reviewed individually.

When Jenesis Living is not the best fit, we communicate honestly and, whenever possible, help connect partners with appropriate community resources.

Our referral process

From first inquiry to ongoing support, we work collaboratively to ensure each individual receives thoughtful, person-centered consideration.

  1. Referral Inquiry Received

    A case manager, guardian, Managed Care Organization, healthcare professional, or other authorized referral partner contacts Jenesis Living to discuss a potential referral.

  2. Referral Acknowledgment

    We acknowledge the inquiry promptly and identify any additional information needed to begin a thoughtful review through an appropriate channel.

  3. Initial Review

    We review eligibility considerations, support goals, and overall service fit within Home Based Habilitation and our current residential models.

  4. Information Gathering

    When needed, we collaborate with referral partners, families, guardians, and members of the care team to gather information through a process appropriate for confidential details.

  5. Meet and Greet

    When appropriate, we arrange meetings with the individual, family, guardian, and care team to explore whether Jenesis Living may be a thoughtful match.

  6. Admission Decision

    We communicate the decision and discuss next steps—whether services begin with Jenesis Living or another community resource may be more appropriate.

  7. Service Begins

    When services begin, we continue communication and collaboration among members of the individual’s support team.

Throughout every referral, you can expect respect for person-centered planning, transparency, prompt communication, professional collaboration, and honest discussions regarding service fit.
